Our mission is to “partner with Illinois families and communities to help children and youth with special healthcare needs connect to services and resources.”

Our website identifies areas of care coordination as How We Help. Each of these care coordination areas has a clickable page to describe how we can help.

To really understand the impact of our care coordination, read some of our Family Stories. These are real accounts from families we serve.
