Family Surveys

We want to hear from you!

Our care coordination focuses on each participant family’s unique needs and goals. We’ll ask for your feedback along the way to make sure you receive the right support.

Our family surveys are an important and easy way for you to share your input.

We send a series of surveys at certain points in your child’s journey with the Division of Specialized Care for Children (DSCC).

These surveys include:

  • An initial survey 60 days after your enrollment.
  • An annual survey sent during February each year.
  • A series of surveys focused on transition when your participant turns the following ages: 14, 16, 18 and 20.
  • An exit survey when your participant leaves our program.

You will receive these surveys via email or mail.

Each survey is short and should only take a few minutes to complete. These surveys are optional, but we encourage you to share your feedback.

Your Feedback Matters!

Your response is important to us and can help strengthen our services for families across the state. We used family input from our 2024 annual survey to make the following improvements and updates:

  • Train our care coordination teams to better understand the family perspective and tailor services to your unique needs and wants.
  • Educate families about the benefits of our DSCC Family Portal to easily communicate and share information with your care coordination team.
  • Launch NurseNet, a new online tool to help families connect with available in-home nurses.
  • Offer free webinars on important health insurance topics affecting families.
  • Improve our language access to better support our participants’ diverse backgrounds.
  • Reorganize our Chicago offices to better serve the Chicago community.
