DSCC Fact Sheets Resources
It’s important for everyone involved in your child’s care to understand their needs and what makes them unique. The Division of Specialized Care for Children (DSCC) has created “All About Me” Pages that you can personalize to help anyone on your care team and beyond get to know your child. These free templates highlight what works best for your child, what they like and dislike and who they are as a person.
There are a variety of designs to choose from to help you capture your child’s personality. You can share them with doctors, nurses, schools, other caregivers and more. Templates are available in both English and Spanish.
Certified Nursing Assistant (CNA) Resources
This tip sheet from the Division of Specialized Care for Children (DSCC) aims to help guide parents and caregivers through the steps to become a certified nursing assistant (CNA). It links to several websites and provides general guidelines.

Cleft and Craniofacial Condition Resources
HealthyChildren.org, the parenting website of the American Academy of Pediatrics, provides articles and information for parents and caretakers of children with a cleft lip, cleft palate or craniofacial disorder.
ClinicalTrials.gov is a place to learn about clinical studies from around the world.
Coloring Pages for Caregivers
UIC’s Division of Specialized Care for Children has created adult coloring pages to help caregivers relieve stress and practice mindfulness. Four designs are available in English and Spanish:

ComeUnity is a parent support website with resources on parenting children with special needs, adoption, child development and multiple lists of groups and organizations providing information on specific topics or areas.

Coming Out: Information for Parents of LGBTQ Teens
HealthyChildren.org, the parenting website of the American Academy of Pediatrics, provides parents of lesbian, gay, bisexual, transgender, and questioning (LGBTQ) teens information on understanding, supporting and remembering that each child is unique and will have their own experiences and feelings along the way.
“Coming Out: Information for Parents of LGBTQ Teens” provides help for parents and children navigating the many aspects of acceptance.
The Secretary of State’s Office offers an Illinois Department of Human Services (DHS) Communication Wallet Card to help notify law enforcement that a person may have difficulty communicating. Applicant’s may request the wallet card at any Illinois Secretary of State Driver’s License Facility.
The wallet card is available to notify law enforcement in advance that a person may require alternative methods of communication in order to respond to and communicate with an officer.
Community Housing Advocacy & Development (CHAD)
Community Housing Advocacy & Development (CHAD) is a nonprofit corporation that provides quality, affordable, long-term rental housing to income-qualifying, hardworking individuals and families who struggle to support themselves.
CHAD has over 500 housing units of all property types in 30 communities across DuPage, Kane, Will and portions of Cook counties in northeastern Illinois. Properties include apartment homes, condominiums, duplexes, townhouses, and single-family homes. CHAD offers below-market rents to income-qualified applicants.
Community Nurse Health Center provides primary medical care and dental care. Services are available to all families and individuals regardless of your insurance status or ability to pay. A sliding fee is available for low-income and uninsured individuals. Medicaid is accepted.
