Nurse-Family Partnership of Sangamon County
Nurse-Family Partnership of Sangamon County, provided by Southern Illinois University School of Medicine, serves first-time parents in Sangamon County to help improve pregnancy outcomes, child health and development, and economic self-sufficiency.
Nurse-Family Partnership (NFP) creates positive outcomes through nurse home visitors who work with families from pregnancy until the child’s 2nd birthday. Individuals may qualify for NFP of Sangamon County if they are less than 28 weeks pregnant and have financial, social or health risk factors such as:
- Loss of household income or an inability to pay for living expenses
- Chronic health condition(s) or mental health concerns
- Domestic violence
- Transportation or access to care difficulties
No proof of income is required for enrollment. Referrals can be made by providers, nurses, community services and individuals by calling (217) 545-0175, emailing at siunfp@siumed.edu or using IRIS (integrated referral and intake system) or Touch Works “SIU Nurse-Family Partnership”
Our Resilience is an independent, not-for-profit organization dedicated to the healing and empowerment of sexual assault survivors through non-judgmental crisis intervention counseling, individual and group trauma therapy, and medical and legal advocacy in the greater Chicago metropolitan area. Our Resilience addresses the entire spectrum of sexual violence by addressing harassment, abuse, prevention, public education and institutional advocacy.

PACER’s National Bullying Prevention Center
PACER’s National Bullying Prevention Center provides resources and information regarding the bullying and harassment of students with disabilities.
Prairie State Legal Services (PSLS) offers free legal services to people living in poverty, with disabilities, aged 60 and older across 36 counties in Northern and Central Illinois. PSLS provides legal help to protect survivors of domestic violence and help people facing issues impacting financial stability and serious housing issues.

Project SUN in Kankakee County
Project Strong and Unified Network (Project SUN) is a family-driven, youth-guided and community-based system of care in Kankakee County. Project SUN provides resources, training and community education about children’s behavioral health and other child and family wellness topics. Its services include:
- Helping families navigate their children’s mental, emotional and behavioral health systems of care through trained staff and volunteer Community Navigators
- Supporting families on their journey by training peer parent mentors and companions and offering appreciation events for caregivers
- Assisting parents and youth to gain the skills necessary to actively participate in their care planning and delivery
- Offering professional development training for mental, emotional and behavioral health service providers
- Providing community education with a focus on children and their families

Puentes de Esperanza (Bridges of Hope)
Hoyleton Youth & Family Services‘ Puentes de Esperanza (Bridges of Hope) serves Spanish-speaking families in Clinton, Madison, Monroe, Randolph and St. Clair counties. Puentes de Esperanza connects Hispanic families to community resources, including interpretation and translation, finding employment, immigration and citizenship information and other resources.
Puentes de Esperanza also:
- Partners with the Illinois Coalition for Immigrant and Refugee Rights (ICIRR) to help immigrants know their rights
- Hosts mobile events with the Illinois Secretary of State Department of Motor Vehicles

The Arc’s Talk About Sexual Violence project provides materials and practical tools to help health care professionals, people with disabilities, and their families and supporters learn about the depth of the problem, support victims and work to prevent it.
Talk About Sexual Violence seeks to empower survivors with disabilities in the reporting process and give health care professionals the tools to more effectively identify victims, connect them to appropriate services, help build their self-advocacy skills and better prevent sexual violence.
In addition to the “Starting the Conversation Toolkit for Self-Advocates,” put together by Self-Advocacy Resource and Technical Assistance Center (SARTAC), resources include short training videos for healthcare professionals, tools to help guide conversations and more.
Rincon Family Services offers a wide range of support to individuals and families across Chicagoland. Programs and services include counseling, case management, violence prevention, education, and substance use prevention and treatment services.
Rincon Family Services community programs also include:
- Rental assistance and eviction prevention
- Immigration integration
- A construction pre-apprenticeship program and more

Safe Families for Children hosts vulnerable children and creates extended family-like supports for desperate families through a community of devoted volunteers who are motivated by compassion to keep children safe and families intact.
There are several locations in Illinois.
Safe Harbor Family Crisis Center
The mission of Safe Harbor Family Crisis Center is to support and respond to the needs of survivors of domestic violence and their dependents in the effort to avoid further abuse. The center is based in Galesburg and serves victims of domestic violence in Knox County.
Safe Harbor’s services include a crisis hotline, court advocacy, support groups, social service referrals, clothing and food, and children’s programs.
