New Approval Process for Respite Nursing Services

New approval process for respite care for individuals enrolled in the MFTD waiver begins on Sept. 1

Families enrolled in the Medicaid Home and Community-Based Services Waiver for Those Who Are Medically Fragile Technology Dependent (MFTD waiver) will see a new approval process for respite nursing services.

This change will help make respite nursing services more respectful of your wishes.

Starting Sept. 1, 2025, nursing agencies must get your permission and official approval from the Division of Specialized Care for Children (DSCC) before providing respite care to your child. This means a nursing agency or respite facility cannot use your respite hours unless you ask for them and agree to the plan.

Why This Change Is Important

We are making this change because families shared concerns that agencies and/or facilities used their respite care hours without their knowledge or permission. We want to make sure:

  • You are always in control of when and how your family uses respite care.
  • Respite services follow Medicaid and MFTD waiver rules.
  • You know exactly how many respite hours are used and when.

What You Need to Know

  • Each year, families in the MFTD waiver get 336 hours of respite care.
  • Respite care is only available if the legally responsible adult (LRA) asks for it.
  • Nursing agencies cannot use respite hours without your clear permission.
  • If you request respite, the agency must fill out a form and get approval from DSCC before care begins.
  • Respite hours don’t roll over to the next year and can’t be used as overtime.

If you think your child needs respite care, talk to your nursing agency about how many hours you want and when you need them. Please give them as much notice as possible (at least two business days before you need the care).

Your nursing agency will then contact DSCC to submit your request.

If something comes up at the end of the month and you need extra help, please talk with your nursing agency. They can work with you to request respite.

New Electronic Visit Verification System for In-Home Nursing

Electronic Visit Verification (EVV) is a system that electronically documents the time and location of care that in-home nurses provide.

We have an important update about the in-home nursing services that Home Care Program families receive.

Your nursing agency must now comply with the Electronic Visit Verification (EVV) system. This system ensures accurate tracking of home-based healthcare visits. Your nursing agency may have mentioned EVV to you already.

We want to make sure you understand what EVV is and how it affects you and your nursing agency.

EVV is a system that electronically documents the time and location of care that in-home nurses provide. Using GPS coordinates, EVV automatically collects shift clock-in and clock-out times and locations to access information on care delivery in real time.

The EVV system verifies:

  • The nursing service provided
  • The participant receiving the service
  • The nurse or Certified Nursing Assistant (CNA) providing the service
  • The date and location of service delivery
  • The service start and end times

This information helps ensure there are no care gaps throughout the service schedule. It also helps make sure that nursing hours are billed accurately toward participant nursing hours allocations.

The EVV system overall helps ensure greater transparency and accountability for both caregivers and recipients.

The EVV system allows for better tracking than paper time sheets or agency charting that Illinois Medicaid cannot access. This improved tracking allows for better program quality assurance monitoring and access to quality care.

When your nursing staff clock in or out with EVV, it ensures accurate times are available. Therefore, only the true amount worked is billed against your nursing hours allocation.

There is nothing your family needs to do with the new EVV system.

As a federal requirement under the 21st Century Cures Act, Illinois Home and Community-Based Waiver programs began phasing in EVV on Jan. 1, 2024.

Following months of townhalls, resources, and flexibility, the official compliance period for EVV begins on Oct. 31, 2024.

All nursing agencies enrolled with the Division of Specialized Care for Children (DSCC) will have six months to reach 50 percent compliance and one year to reach and maintain at least 75 percent compliance.

We are helping agencies reach compliance through outreach, education, and remediation plans, if necessary.

A brief survey for Home Care Program families will help us improve service and support

Attention all families in the Home Care Program! We want to know how well your family’s nursing agency is meeting your needs.

The University of Illinois Chicago’s Division of Specialized Care for Children (DSCC) team has put together a short, 10-question survey for families to complete.

We will not share your individual survey responses with anyone.

There is no requirement to share your name or your child’s/participant’s name. Entering this information is completely optional.

We will group your survey responses with those from other families to better understand how rate increases are affecting nurses working in the home.

Our ultimate goal is to improve the services and support available for DSCC participants with complex medical needs.

Please note this survey is only for families enrolled in the Home Care Program who receive in-home shift nursing services.
