The Arc of Illinois‘ Family-to-Family Health Information Center and Illinois Life Span Program (ILS) will host “Healthcare Transition Goals: Youth Voices on Healthcare Transition in the IEP” on Sept. 8 from 12 to 1 p.m.

Successful transition to adult healthcare is key to preparing youth and young adults with special healthcare needs and disabilities for life after high school. Yet, healthcare transition is often overlooked in the Transition Individualized Education Program (IEP).

Join Claire Cook, Title V Transition Specialist at the University of Illinois Chicago’s Division of Specialized Care for Children (DSCC), to learn why healthcare transition matters and how to make it part of your child’s Transition IEP goals.

Using real-life examples, Claire will explore:

• Why healthcare transition is an important part of preparing for your child’s future
• How to include healthcare transition goals in the IEP
• Ways youth and families can prepare for adult health care
• Strategies and resources to help youth build confidence and self-management skills for adult healthcare
