Webinar – “UIC’s Division of Specialized Care for Children: Transition to Adult Health Care”

A free webinar for families of children with disabilities to learn about UIC’s Division of Specialized Care for Children’s resources, services and supports for the transition to adulthood healthcare planning

The Will-Grundy Transition Planning Committee (TPC) will host “UIC’s Division of Specialized Care for Children (DSCC): Transition to Adulthood Healthcare Planning” on May 19 from 6:30 to 7:30 p.m.

This webinar will explore the resources and services available through DSCC.

DSCC Title V Program Transition Specialist Claire Cook and Integrated Program Support Specialist Courtney Kerfoot will provide participants with:

• An understanding of DSCC programs and resources
• Steps for planning your child’s transition from pediatric to adult health care
• Helpful tips, tools and information to help plan for what comes next

The webinar is free and open to anyone who wants to learn more about DSCC and transition-to-adulthood resources.
