The Margaret Hackett Family Program (MHFP) Connect Group will host “How to Obtain Free Pediatric Care Coordination” on Zoom on March 5 from 7 to 8 p.m.
A Care Coordinator is a healthcare professional who acts as a central point of contact, bridging the gap between patients, families and doctors to manage treatment plans. They handle logistics such as scheduling, insurance, connecting patients to resources, and more.
This webinar will highlight how the University of Illinois Chicago’s Division of Specialized Care for Children provides free care coordination for Illinois families of children with special healthcare needs.
DSCC’s Assistant Director of Enrollment and Specialty Care, Gabrielle Schmitt, will provide an overview of DSCC programs and services, and the many ways a Care Coordinator can help families, including:
- Accessing diagnostic tests, specialized care providers, and human service-related professionals
- Answering your insurance questions
- Creating a care coordination plan with your child and family
- Supporting youth and their families with the transition to adulthood
- Providing resources, information and more
Gabrielle will guide parents through the different stages of care and how care coordination can help make a positive difference for your family at every step.
