Family Matters Parent Training and Information Center (Family Matters PTIC) and The Arc of Illinois’ Family Advocate Program will host “From Dinner to Direction: Filling Your Transition Plate” on Sept. 17 from 5 to 8:30 p.m.

Pull up a seat and dig into a wide range of critical topics with Family Matters PTIC and The Arc of Illinois Family Advocate team members Annie Bruno and Jerin Olson.

Families and professionals will learn about key steps and strategies for strengthening transition plans for youth with disabilities.

Topics will include:

  • Introduction to PUNS – What it is and how it connects to waiver services
  • Medicaid Waiver Services – Service options that support community living
  • Charting the LifeCourse Tools – Planning for a good life, not just services
  • Facilitating Conversations About the Future
