Our organization, the University of Illinois Chicago’s Division of Specialized Care for Children (DSCC), helps families throughout Illinois with free care coordination and more.

DSCC Title V Transition Specialist Claire Cook will discuss:

• An overview of DSCC programs and resources for families
• A look at DSCC’s Transition Toolkit 
• How to use transition readiness assessments as planning tools
• How to apply for our programs and more

There will also be time for questions.
