Planning for the future is crucial but can also be a challenging process for families.

Attendees will learn about how special education transition planning fits into the broader idea of future planning and will explore:

• Creating a person-centered plan to document the wants, needs and available resources
• Approaches for effective transition planning for students with an Individualized Education Program (IEP)
• Considerations for families, educators, and support professionals as they collaborate with the student during the transition planning process
