Join this Pediatric Palliative Care Coalition (PPCC) Firefly Chat for “My Child with Medical Complexity Is Turning 18, Now What?” on Dec. 11 from 11 a.m. to 12 p.m.

When a child with medical complexity becomes an adult, there are new and complicated systems to navigate.

This chat will feature PPCC moderator Dana Dombrowski and presenter Jennifer Shuckrow. Jennifer will highlight what her family faced during the guardianship application process and transition out of pediatric-based services for her child with medical complexity.

They will also explore:

  • Making decisions and the challenges families may face
  • Identifying sources of support and resources
