 .m. at the Chicago Heights Family Health Center.\n\nAunt Martha’s Family
  Health Fair offers a mix of medical services and community support to hel
 p families prepare for the upcoming school year.\n\nFamilies can enjoy:\n\
 n 	Kids' games and activities\n 	Browsing booths hosted by local community
  resource organizations\n 	Free backpacks and essential school supplies fo
 r children\n 	Seasonal fresh food baskets for families while supplies last
 \n 	Check-ups to meet school and sports participation requirements\n 	Rout
 ine childhood and back-to-school vaccines\n\nOur Division of Specialized C
 are for Children (DSCC) team members will be at the fair to provide inform
 ation about our programs and how we help children and youth with special h
 ealthcare needs and their families. Please stop by our table!\n\nAnyone ca
 n walk in to enjoy the free activities\, snacks and resource booths.\n\nPh
 ysicals and immunizations require an appointment. To make an appointment\,
  please call (877) 692-8686 or register in advance through Aunt Martha's P
 atient Portal.\n\nFor appointments\, please bring:\n\n 	Your child’s imm
 unization records and any required school health forms.\\\n 	A photo ID fo
 r the parent or legal guardian\n 	An insurance card (such as Medicaid/Coun
